Our Services

Housing Stabilization Program:

This program assists those experiencing housing instability, evidenced by one of the following risk factors:
  • Homeless. An individual or family is considered homeless when they lack a fixed, adequate nighttime residence; or 
  • Currently transitioning, or has recently transitioned, from an institution or licensed or registered setting, or
  • At risk of homelessness.

​Housing Stabilization Services consists of 3 distinct areas of services:
  • Housing Consultation – develop a person-centered plan for people without MA case management services
  • Transitional Services – provide service that locate and obtain housing
  • Sustaining – provide service that supports a person to maintain housing

Veteran Adult Foster Care:

We identify families desiring to pay it forward to disabled and/or seniors veterans looking for a loving safe family environment, who need some daily care, but do not need skilled nursing care.

PA - Personal Care Home (PCHs) and MN Boarding Care Homes (BCHs):

Our PA Personal Care Homes (PCHs) and MN Boarding Care Homes (BCHs) are residences that provide shelter, meals, supervision and assistance with personal care tasks, typically for older people, or people with physical, behavioral health, or cognitive disabilities who are unable to care for themselves but do not need nursing home or medical care to 4 or more persons. While available services vary and are based on the individual needs of each resident, services provided at a typical PCH include assistance with:
  • Eating/drinking
  • Walking/getting in and out of bed or chair
  • Toileting/bowel and bladder management
  • Bathing
  • Personal hygiene
  • Arranging for and managing health care
  • Making/keeping doctor's appointments
  • Assisting with or administering medications
  • Positioning in bed or chair
  • Doing laundry
  • Arranging for transportation
  • Shopping/managing finances
  • Using the telephone/writing letters
  • Caring for possessions
  • Participating in social/recreational activities
  • Using prosthetics
  • Getting and caring for seasonal clothes

Non-Emergency Medical Transportation:

Non-emergency medical transportation (NEMT) includes transportation services offered to patients and healthcare consumers who face extraordinary barriers getting to their medical appointments.
